The following information must be submitted with the grant application in order to be considered:
- Brief statement of organization’s mission, history and goals.
- Description of your project/program and how it will address the need.
- Describe the timeline for the project/program.
- Provide a copy of the current IRS determination letter indicating your organization’s nonprofit, tax exempt status.
