The following information must be submitted with the scholarship application to be considered. Applications may be submitted electronically through the below form, or by downloading the PDF and emailing it to CMCF at cmcommfoundation@gmail.com. You may also mail the completed application to P.O. Box 123, Stanton, MI 48888. Applications must have all supporting materials.
- Brief statement of organization’s mission, history and goals.
- Description of your project/program and how it will address the need.
- Describe the timeline for the project/program.
- Provide a copy of the current IRS determination letter indicating your organization’s nonprofit, tax exempt status.
